Cancellation Policy

Requests to cancel or reschedule must be received within 48-hours of your appointment time, otherwise, you will be charged the full session fee.


Insurance Information

JAK Nutrition is out-of-network with insurance. You can request an invoice that you can submit to your insurance for reimbursement if you have out of network benefits. You can inquire if you have out of network benefits for nutrition by calling the number on the back of your insurance card and asking if you have out-of-network coverage for the CPT code 97803. It is helpful if you can provide them with your diagnosis code or ask if there are any limitations on the coverage. If your diagnosis code is not covered, ask if you can provide them with a letter of medical necessity. Also, ask if your deductible has been met.


Rates and Fees

Rates for services will be discussed during the initial phone call prior to the first session. Payment is accepted by cash, check or credit card. Credit cards are run at the beginning of the month for the past month.  Invoices will also be provided at this time. A $5 fee is added to the session cost if you pay by credit card. 


Sessions

Typically clients meet once a week for 45 minute appointments especially as they are starting out. However, as clients' needs are different, the length and frequency of sessions may change.
